Initiator could not find a match for the initiator task tag in the received PDU. Dump data contains the entire iSCSI header.

Hi, 

time to time we have such error in windows and we it happens that our EqualLogic iSCSI disks fails. We use Initiator name to allow access to SAN and we use HIT. Is any way to troubleshoot these errors. How to be sure when we should go to network team for these errors?
